YOU WILL BE RESPONSIBLE FOR:

•    Responds to requests for care from referring sources within the community in a timely manner.

•    Establishes and maintains positive relationships with referral sources, responding to their requests and concerns.

•    Obtains initial and ongoing referral documentation from physicians, insurers, facilities and other providers.

•    Obtain information to verify insurance coverage and inform patients.

•    Identify primary and/or secondary payer of services.

•    Utilizes home heath record management software.

•    Participate in establishing the plan of treatment through coordination with the patient’s physician by obtaining specific orders for care.

•    Explain the function of the health team members who will implement the plan of treatment to the patient.

•    Coordinate the start of care activities with the Scheduler and or Supervisor.

•    Function as a resource for interpreting Medicare  and non-Medicare reimbursement requirements  and home health guidelines.

•    Communicate pertinent information and instructions from specific physician’s orders to health team members, including completion of the interdisciplinary referral.

•    Participate in committees as requested.

•    Document intake coordination activities according to department procedures.

•    Assist in making appropriate referrals for patients not accepted for care.

•    Enhance personal and professional growth through assigned educational offerings and individual research and reading.

•    Adhere to all department policies, procedures, standards of patient care and department guidelines.

•    Adheres to HIPPA and patient information privacy requirements

•    Complete other tasks as requested by supervisor.

•    Other duties as assigned.

Qualifications
QUALIFICATIONS:

•    At least two (2) years of experience in a health care related field.

•    Excellent working knowledge of Medicare covered home health care and other insurance program regulations and guidelines regarding home health care.

•    Skill to handle difficulty phone calls, make quick decision and deal with stressful situation.

•    Must be detail oriented, multitask, and efficient team player with good computer and typing skills

 
PREFERRED:
 
  • Knowledge of hospitals discharge process.
  • Additional languages.
  • Successful completion of healthcare related certificate program such as Certified Nursing Assistant, Medical Assistant. 
  • Certification in healthcare related field.
Job Summary:

This position coordinates receipt of referrals within the agency by conducting an initial phone assessment of the patient for appropriateness of and acceptance of the referral for home health services. The clinician who performs the admission will determine if the patient is admitted based on the initial assessment. This position is responsible for collecting patient clinical and demographic data, insurance or payer information, physician orders and arranging for support services.
